Creating an Employee Survey: What Are the Key Factors for Rating a Good Employer?

I have to form a questionnaire regarding the parameters considered most important for employees when rating a good employer. It's very urgent. Please give me suggestions on the above and how I can phrase indirect questions in the questionnaire.

8. How does communication take place in the organization in the following cases:

• Policies taken by top management

• Employee suggestion and feedback
