How Do I Write a Polite Yet Powerful Salary Increment Request to My Manager?

I want to write an increment & promotion letter to my Branch Manager as he agreed to increase my salary after completing 1 year. I had a talk with the Branch Manager; he said to write the email, so please help me in writing the mail. Till now, there's been no way. Please help me in writing the Increment Letter in a polite yet powerful way:

1. I am a hard worker and have proven myself.
2. I have a small salary.
3. My designation is Front Desk Officer, and I also work as an Admin Executive and AutoCAD Operator.
4. I am staying with my family.

Please consider the above points and assist me in crafting the increment letter.

Your earlier action will be appreciated.

I have been appointed as a receptionist, and the office timing is from 9:00 to 5:00. Initially, the joining time was not fixed, and I was handling the entire department by myself. Initially, there were two people working in the reception, one as a receptionist and the other as an assistant. However, when I joined, I was the only person handling both the reception and the Mediclaim (TPA Department). Therefore, I would like to request the director for a promotion and a salary increment. Unfortunately, the manager has not responded to my request.
