Need Help Creating a New Employee Agreement Document for First Day Signatures

In my company, I have been asked to prepare a document for new employees which should indicate that they are aware of each rule and agree to the same. This document will be signed by the new employee on their first day. Could you please provide me with a format or sample for this document? I need to prepare it urgently.
