Full And Final Payment Format For An Employee

i want to know about full and final settlement processor and i need a format in excell format for full and final settlement with a employee regarding gratuity and provident fund

Can a company hold performance link salary while doing full and final settlement ? I have left previous company with complete formalities done , full and final settlement is expected to happen within 45days, which is not yet done, what is rule in this ref?
