Joining Process Query

I introduce myself as a new entry in this site,please help me to get a joining report ,ie once the employee in given the offer letter ,on the day of joining he/she needs to fill in a report before taking up the responsibilities.

Joining process is a part of the New hire Integration. It includes filling of forms, submission of documents, introduction to the process and induction program.
