hi.. i need to design a questionnaire to filled by the candidate before interview so that we can have a basic idea about that candidate before interviewing..
like:

Qualification:
Experience:
Present salary:
Expected salary:
so please help me out with your brilliant ideas & tell me what all can i add..

I do not have a format but I would like suggest that - you anyways have the candidates CV and its better to ask less number of questions and what you need to know about the candidate.

If u still need to design the questionnaire find the mentioned details. I have the format but in arabic language hence i will suggest you some points as follows;

Qualification: Educational, Professional , Technical, etc.
Employment History:
Current & Expected Salary (CTC):
Reason for Change:
Achievements (if any):
Time Period to join:
Marital Status:
Others (depends on location, industry you are associated with) and specially Overseas:
Technical Assessment : (if position for tech people)
Passport Details (Number, issue date, expiry date):
Date of Birth:
Criminal Record:
Ready to work shift (if needed):
Details about Parents (father & mother - as some contry visa processing it requires)
Contact Details (local and home country)
Bottom u can leave space for concern deptt. comments with the headline OFFICIAL USE ONLY and FINAL DECISION / COMMITTEE DECISION

the purposes of invervies is to find out what we want, and how can candidate help the establishment, in other words we must watch their abilite. here is some questions which can help you for looking the best candidate.
1. Ask him about himself.
2. what is his greatest stengths
3. What is his greatest weaknesses.
4. Why should I hire you.
5. where do you see yourself three years from now.
6. Why do you want to work with our company.
7. Why have you been out of work so long.
8. Tell me about a situation when your work was criticized.
9. Can you work under pressure.
10. What makes you angry
11. How do you feel about working nights and weekends.
I things these type of enquires will be useful to you to know about the candidate.

Hi Neha,
Please find the attached format for Interview assessment form. I hope it will help you. You can modify the same as per your requirement.Give the same format to candidates when they come for interview & let them fill their personal details/ educational qualification/ strengths/ weakness/ references.In same form you can also write your comments regarding the candidate & if you find him suitable as per your requirement, you can recommend him/ her for second round of interview with concern department.
