How Do You Create a Fair Performance Appraisal Form for Unskilled Laborers?

In my organization, we have more than 75 unskilled laborers, and I have to create a performance appraisal form for them. What could be the contents of this form? Could anybody help...

An appraisal of laborers should be no different than for an executive: What are the responsibilities? How do they fit into the overall scheme of the department's goals (the department's fit into the organization's goals)? How can they improve efficiency (training/schooling, equipment, methods, materials, and the like)? What are professional/personal goals? How do you expect to achieve them? What time frame? Areas where expectations have not been met? What kind of corrective action is necessary? Where have expectations been met? What rewards have been presented? How has job performance been seen by other "stakeholders"?

Just a few thoughts.

I am working in an NGO in which I have to do performance appraisals for the first time. Kindly help me out with forms.

The attached performance appraisal is used by the construction industry I am working in for the unskilled category of workers. Hope this will suffice.
