Site Staff Attendance Format

Hi,
This is Kavitha here from Bangalore, i am working in a construction company. I am not able to get the attendance report from site staff. Can anybody please help in forwarding the site attendance format where the customer/client signature with remarks to be mentioned.

Am working for M/s Millennium Pools and the strength of employees is 40 on rolls. I am in need of site attendance format where the customer will also sign in the format.
