Required: POLICY ON PERFORMANCE APPRAISAL

I am currently working on creating a Performance Appraisal Policy, please share your Performance Appraisal Policies if you can ... that will be very useful for me.....you may even give guidance and suggestions to me on this. IT'S VERY URGENT!!!!

You need to decide policy based on your company need and type.
What kind of system are you planning to implement?
Half yearly/Yearly/Quaterly..............
Look at your industry trend...............

This is Rekha and i am new in HR.
I have to make a performance appraisal policy.So i need yours help in this.
As i said i am new in HR. I do not know more about the policy making, its my first time. So please help me out and solve my problem. I am really very glad to know your points on it. Please help
