Need Help Crafting a Confirmation Letter for Call Center Agents' Promotion and Increment

I have just joined an international call center as an HR professional. I need a good format of a confirmation letter to give to those agents who have completed 6 months in our company, reflecting their increment and promotion. Hope you guys will help me in this regard.
