How Do I Write a Proof of Residence Letter for an Employee? Urgent Help Needed!

I need help urgently, please. I have been approached by one of my staff requesting that I write him a proof of residence letter from the company so he can use it at the bank. I am kindly asking if there is anyone who knows how to design/write something like that to help me.

TO WHOMSOEVER IT MAY CONCERN

This is to certify that Mr. Emanuelo, employee number M1055, has been employed in our organization since 05th May 2010 as Manager of Human Resources. His residential address, as per company records, is 45 Cathedral Road, Chennai - 600002.
