Employees Provident Fund Form No. 10 C & Form 19 Pdf Download

Can anyone pls send me sample forms of Form 10C, Form 19 and SSN ASAP?
pls fill up any faulty info..i jus want to get an idea how to fill up these forms...

I am from Tamil Nadu I worked as lecturer in an Institution for 1+ Years . I got my Form 19 Settled from the PF office but Form 10c Returned to my Institution.What would be the reason for it? Could u please guide me how to solve this Problem.

I have some confusions in the PF challan submission, Seniors kindly help me,
1. what is A/c-1,A/c -2,A/c-10,A/c-21, A/c-22.
Please help me how much should be amount in each account,For example : suppose I had deducted total Rs.76924/- from employees which includes employer and employee share equally.
My doubt:
a) how much amount should be in A/c -1(what percentage and from whose share employee/employer?).
b) how much amount should be in A/c -2(what percentage and from whose share employee/employer?).
c)how much amount should be in A/c -10(what percentage and from whose sahre employee/employer?).
d)how much amount should be in A/c -21(what percentage and from whose sahre employee/employer?).
e)how much amount should be in A/c -22(what percentage and from whose sahre employee/employer?).
Please help me, the PF is being a bottle neck for me now.
Its my sincere request to the seniors kindly help me to know this.

i just want to knw abt SSN form. in my company they told me that u have to fill dis form. so plz tell me from where i got dis form. is dis compulsary to fill it for PF claim.

A memeber joined in my organization at the age of 55, after a nine years of service, he resigned and applied for pension. But at that time Pf authorites told this person will not be able to draw pension?
To get his pension amount , we have to produce form 3A of him. Do we have any other option ?
