Struggling with Pay Structure Calculations: Can You Help Me Figure Out the Formula?

Hi everyone, can someone help me with the computation of the pay structure? My boss gave me the company's existing design of the pay structure for review. The problem is it only comes in values. There are some formulas, but I don't know where to base my computation. I have attached a sample copy. Please help me derive the formula.
