How Can I Create an Effective Competency Mapping Questionnaire for Junior Managers?

I'm a student of MBA (HR) doing my summer project in a KPO firm. I need to develop a competency mapping questionnaire which I will administer at the junior managerial level to compare their perceived competencies with their performance results. Please help me out.

Create your competence dictionary - look at the position while defining the competencies (technical + behavioral). You can also conduct Behavioral Event Interviewing (BEI) with the best performers and gather feedback from managers for the same.

Based on that, ask the incumbents for self-rating, providing examples of indicators for each competency if necessary.
