Employee Satisfaction Form - Doc Download

Hi,
I am working in a CA firm and I want to survey employee's expectation from management. Can any one pls provide me the form or plan to know the employee's satisfaction, from which we can know the needs of employee or expectation from management.

Does CA stand for Cost Accountants?
How many staff members have you got and are they all accountants?

CA Stands for Charted accountants. we have 20 staff members all are charted accountants and 40 members who are going through their articleship.

There are two ways of doing it depending upon the purpose. If you need a quick answer, find a questionnaire used by someone (for example, see the paper at ScienceDirect - The British Accounting Review : Management accounting information and the needs of managers : : Perceptions of managers and accountants compared)
Or you can start from first principles and ask the 40 members going through their articleship to list three good points of their managers and three points that they would like changed/improved in the order of importance. Then based on their answers prepare a questionaire using Likert Scale.
