Documents For Withdrawal Of EPS

Can Anybody tell me what are the documents required along with 10C for claiming EPS amounts.
And what are the documents required for Those are completing 10yrs jobs.

Form 10C : Documents required are copy of Bank Pass Book or Bank Statement duly signed by applicant
Less then 10 Yrs opt you not willing to accept scheme certifcate and for more tahn 10 Yrs opt for willing to accept scheme certifcate
Form 13 : No ducments required except Form 13 sould be duly completed and signed by present employer

Form 19 is also required to withdraw for EPS. form 13 is required to fill when you want to opt for the transfer the amount from an existing pf account to a new pf account.
