I have a few suggestions to what to do with the forms they provide you
1.Once you have created your form load it into your company website as word or excel document & let your vendors do the filling of the form designate one of your employees to do the data collection in the back end
2.Or create a web page with form fields in it for vendors to fill in their information and at the end of each business day make sure your update your data base from your website.
