I have a few suggestions on what to do with the forms they provide you:
1. Once you have created your form, load it into your company website as a Word or Excel document and let your vendors fill out the form. Designate one of your employees to handle the data collection on the back end.
2. Alternatively, create a web page with form fields for vendors to enter their information. At the end of each business day, ensure you update your database from your website.
