You can deisgn 1 on the basis of few inputs mentioned here -
1 - Against each goal person needs to assess on -
- Goals achieved / missed
- Process of achievement . Eg - Process improvement, Trainings attended ect.
- Ratings can be put against each goal and calculated as avg. at the end or one overall rating can be asked for.
- mention if done something beyond goals mentioned.
2 - You can ask for brief writeup on a free flowing doc.
