You can design a system based on the following inputs mentioned here:
1. Against each goal, the individual needs to assess:
- Goals achieved/missed
- Process of achievement (e.g., process improvement, trainings attended, etc.)
- Ratings can be assigned to each goal and calculated as an average at the end, or an overall rating can be requested.
- Mention if something beyond the goals mentioned has been accomplished.
2. You can request a brief write-up in a free-flowing document.
