You have a very impressive report format; however, I think you need to consider the following points in your report:
• Number of Near misses
• Number of Incidents Reported
• What corrective measures have been taken
• What preventive measures have been taken
• Number of meetings held by the HSE committee (if you have one)

You can add these notes to your report to make it perfect.
