We will be having our half-yearly appraisals in June, and I have also prepared the form for the same. However, I need to make a few changes in order to make it more effective. Can someone suggest a few points that must be present in any performance form?
The points which I included are:
1) Personal details (employee name, designation, date of joining, etc.)
2) Main roles and responsibilities
3) Key performance areas
4) Job skills and competencies
5) Soft skills and competencies
6) Achievements in the assessment period
7) Strengths and weaknesses
8) Training & development needs (areas of improvement)
