Skill Set & Behavioural Assessment Sheet - XLS Download

I have uploaded the skill set & behvavioural assessment sheet, do let me know if the same is complete, requires some addition/deletion of skills or behavoural indicies.
i am planing to integrate the same with Performance appraisal form and also give a graphical presentation of their performance.

Team Player, Ready to take ownership, assertice etc
