Seeking Feedback: Is Our Skill and Behavioral Assessment Sheet Comprehensive Enough?

Dear All,

I have uploaded the skill set and behavioral assessment sheet. Please let me know if it is complete, or if it requires any additions/deletions of skills or behavioral indices. I am planning to integrate the assessment with the performance appraisal form and also provide a graphical presentation of their performance.
