Need Help with PF Closure: Confused About Filling Form 10c?

Hi,

I want to close my PF and I have some doubts in filling the form 10c. Please answer my below questions:

1) What option do I have to select for point 8 (Are you willing to accept the scheme)?
2) Under Advanced Stamp Receipt, do I need to sign and send the form to my ex-employer?

Question 1: What option do I have to select for point 8 (Are you willing to accept the scheme)?
A) If you want EPS (Employee Pension Scheme) benefits, then tick YES; otherwise, put NO. If you select Yes, you will receive a scheme certificate for the pension. If you select No, you will receive the full amount from EPS & EPF.

Question 2: Under Advanced Stamp Receipt, do I need to sign and send the form to my ex-employer?
A) Yes, you can sign and send it to your ex-employer.
