Please Clarify My Doubts Regarding FORM 10c

Hi ,
I want to close my PF and I have some doubts in filling the form 10c.
Please answer my below questions:
1)What option i have to select for the point 8 (Are you willing to accept scheme)
2)Under Advanced Stamp Receipt,do I need to sign and send the form to my ex-employer.

Question 1) What option i have to select for the point 8 (Are you willing to accept scheme)
A)I f you want (EPS Employee pension benefits ) then tick YES other wise put NO.
If you select Yes u will get scheme certificate for pension
If u Select No you will get full amount from EPS&EPF
2)2)Under Advanced Stamp Receipt, do I need to sign and send the form to my ex-employer.
A) Yes u can sign and send it to ur ex- employer
