Struggling to Find the Right Identity Certificate Format for PAN Card Update – Need Help!

Hi All, I need a certificate of identity (or identity certificate) letter format—with a photo—for PAN card address proof correction. The letter needs to be signed by a Tehsildar. I checked on the internet for this template format, but none of them (including TIN NSDL) have the sample form for this. The concerned person in the taluk office informed me to get the letter format with details without which he cannot sign and issue the letter. I am totally confused about this and have no idea how the format should be. Could anyone kindly help me with this? The letter should contain details as follows:

1) Name—self
2) Father's Name
3) Date of Birth
4) Residential address

Document Format for PAN Card Address Correction

I have attached a document format used to obtain the PAN card for address correction. I hope you can make minor changes as per the requirements for the bank account. Please note that this document needs to be attested (signed) by a registered Gazetted Officer.
