Need PDF Versions of Form J and Form X for Maternity and Minimum Wages Acts in Tamil Nadu

Could you please provide me with the PDF format of Form J (Abstract of the Maternity Benefit Act, 1961, and the rules thereunder) - (Rule 15 of Tamil Nadu Maternity Benefit Rules, 1967) and Form X of the Minimum Wages Act (Rule 22 of the Minimum Wages Tamil Nadu Rules).

Please find attached the abstract of the Maternity Act in Form K. This form is to be displayed, and the necessary forms are to be obtained from employees as per the Maternity Benefit Act, 1961.

Are these forms and abstracts applicable for Maharashtra?

One query: During the first pregnancy, suppose it's a miscarriage, how many leaves are permitted, and are these leaves deducted from the total number of leaves available during the second pregnancy?

Please guide.
