Struggling with Appraisal Forms? Seeking Advice on Going Digital and Timely Submissions

We have an appraisal form that does not capture behavioral aspects. I am in the process of redesigning the form. I also face problems with forms being submitted on time. I was thinking of making this system online. Can someone help me? I need to put this on the intranet or get a cheap off-the-shelf software. I really need some help and guidance to implement the same.

Desirable personal traits are to be rated in the performance appraisal, which differ as per the role and job description. I can suggest some traits if you give info on the role and job description.

In order to help you redesign your performance appraisal, please send me the one you're using and check what the HR manual says about the ratings. Also, don't forget to provide me with the traits you are evaluating in your employees.

Please note that the most effective way to evaluate employees is by using the appraisal by objectives method. This includes setting core competencies decided by the management to evaluate employees against. Additionally, remember to incorporate training and development aspects.
