I Lost Original ESI Challans

Hi Everybody,
Recently few days back i hve lost my ESI original CHallans somewhere. So, now i need to file half yearly filing in ESI Office.
Can anyone suggest me what can do and to whom we need to meet.

please you go to the bank and tell him to this metter
bank show their record and make neceray note against your chhallan paid in our bank

Dear Vinay,
Prepare a letter on co. letter head to Bank(in favor of bank, where you have deposited the challan) with all the details having chq no. amount and date etc. and bank will give u a certificate of ESIC contribution, which you can use as a challan to anywhere. Bank will charge a nominal amount for the same.

With addition to the above said suggestions also make a habit of scanning all the original documents time to time (as soon as you get it )and keeping in seperate folders and keep a good quantity of xerox of it, it will help you in urgency.

Hi Vinay,
Please go and approach the Bankers where u have deposited the ESI Challan along with ur Cheque Particulars like Cheque Amount, Cheque No., Cheque dated, Drawn on which bank with your covering letter and they will do the needful.

Dear Vinay, Don't worry about that you just approach the concerned Bank, where u have deposited the amount(cheque). They will give a certificate to that effect saying that u have paid an amount of Rs......... in to ESI A/c.No. I on your ESI Code No............ towards the contribution for the month of................
A copy of the same may please be attached in lieu of the lost challan... Further two things u have to do:
i. Write a letter to the Regional Office concerned alongwith the details of lost challans and enclosed the Bank Certificate;
ii. U can keep ur Bank Statement, where the amt is debited towards ESI Contribution (this will be one kind of evidence from ur records......

You HAVE TO GO TO BANK, WHERE DID YOU SUBMIT ESI CHALLAN, ALONG WITH COPY BANK STATEMENT, COVERING LETTER FOR REISSUE OF ESIC CERTIFICATE AND AUTHORITY LETTER.
THEY WILL CHARGE APROX RS 150/- EACH NEW STATEMENT FOR ESIC. YOU HAVE TO CONTACT TO THEIR CLERK WHOSE HANDLE THE CHEQUE CLEARING.
