Need Help with Full and Final Settlement Format for Resigned Employees?

I need the format for the calculation as well as the letter to be given to the employee. I would also like to know if there is any clause for a specific period to make the final settlement for a resigned employee.

Please help me to understand the calculation:

1. You have calculated basic/31*5. Is 5 the number of leaves to be encashed, or is it something else? (Actually, we should divide by 30 days for all calculations, not 31 days.)
2. The same goes for HRA.
3. For leave encashment, you have used the formula as basic/31*2.5. Please help me understand this.
