How Do You Handle Non-Performance Warnings or Terminations During Probation?

Can somebody suggest a good format to either terminate or warn an employee of non-performance during probation and confirmed employees?

I'm providing a format. This is just a template that I prepared after reviewing your post. Feel free to make any changes you wish.

Dear Mr. [Recipient's Name],

We have observed that you are not performing well in the tasks assigned to you, neither properly nor efficiently. This serves as a warning. You will have one month from the date of receiving this notice to demonstrate improved performance. Failure to do so within this period will result in termination of your duties.

Dear Sir/Madam,

I am writing in reference to our recent conversations regarding your performance at work. Despite receiving close coaching and training, you have not been able to deliver the expected output.
