Soft Copy Of All Forms And Returns Under The Gratuity, Bonus

Hey all,

I am looking for forms and annual returns under the Gratuity Act, Maternity Benefit Act, Contract Labour Act, Payment of Wages Act, leave forms, pay sheet as prescribed under minimum wages, attendance register, etc. Can anyone provide me with these formats?

I was employed with a company in Chennai for 1 year and 7 months in the year 2000-2001. I then moved to another company in Bangalore and requested to have my PF transferred. When I quit that company in 2004 and received my PF from the Bangalore company, I realized that my earlier PF had not been transferred.

I want to know if I can proceed and request the company in Chennai to process my PF withdrawal, considering the long gap that has elapsed.

Please advise.

I think you should call them and ask for the reason for the non-transfer of funds. It doesn't matter, so long.
