However, the ICICI branch refused to accept the cheques stating that the bank account number did not start with double zero (00) as an initial to the 10 digits of my existing bank account number.

Now, one of the officers at the Peenya PF office has advised me to fill out a reauthorization form. I kindly request your assistance in this matter and ask for the reauthorization form from the PF office in Peenya, Bangalore.

The same kind of situation happened to one of my friends, and he was asked to submit a P.F. reauthorization form by the P.F. office, and the problem was solved. I have posted the authorization form earlier upon one of our site HR friend's request. However, according to your request, I am attaching the P.F. authorization Form once again with the changes to be made in this form as per your P.F. office address where you are going to submit. Since it is a Bombay-based form (the form can be used across the country), you need to take a signature from your ex-employer as well as sign (under the "Yours Faithfully" column) and submit the same to the respective P.F. office.
