Pls let me know some hints about filling up for 10C and 19. and also send me a sample-FILLED 10c and 19 form.

Dear Friend,
no need of sample formats to fill form 10 C and 19 it easy to fill .
you can fill the both forms your self easily. you have to write your Name , Father's Name, Date of Birth, Date of Join, Date of Leaving, Address and Bank Details etc.
and you need to fix two 1 Rupee Revenue stamps in appropriate places and you have sign respective places.
in Form No. 10c there is a question stating that are you willing to accept the scheme certificate ?
here you have to tick No else they will issue a certificate this certificate has to retain until completion of 58 years of age.
if you read both forms twice I am sure you can easily fill .
after filling of this forms forward to your ex-employer. After completion of their formalities like signature, filling of from 3A etc.. once you received the same from them you can submit to P.F office.

No revenue stamp is needed as at present all epf and eps transactions are made online. The pf office has also published a circular.

The dl is for region sign so you have not to worry about it. You have to write the pf no. As provided in uan site. Your pf no. Also mentioned in your accounts slips.

Hi nagendra.u, in 10 c those who have not completed 10 years they have to withdraw their entire eps but as per the table -d calculation. Those who have completed 10 years as per eps scheme they are eligible for pension but after after completion of 58 years. For them the office has insert a column on 10 c for scheme certificate. Where you have to mention your wife's name, childrens name as the nominee their age/dob and relation. On the basis of that the pf department has issued a scheme certificate which you have to keep till the age of completion of 58 years for pension. After you your dependent are eligible for pension.
