Struggling with Form 10C and 19? Need Tips and a Sample to Get Started?

Please let me know some hints about filling out Form 10C and 19, and also kindly provide me with a sample of a completed Form 10C and 19.

Dear Friend,

There is no need for sample formats to fill out Form 10C and 19 as they are easy to fill out. You can easily fill out both forms yourself. You need to provide your Name, Father's Name, Date of Birth, Date of Joining, Date of Leaving, Address, Bank Details, etc. Additionally, you need to affix two 1 Rupee Revenue stamps in the appropriate places and sign in the respective spaces.

In Form No. 10C, there is a question asking if you are willing to accept the scheme certificate. Here, you have to tick 'No', otherwise, they will issue a certificate that needs to be retained until you reach the age of 58.

If you read both forms carefully, I am sure you can easily complete them. After filling out these forms, forward them to your ex-employer. Once they have completed their formalities, such as signing and filling out Form 3A, you can submit the forms to the P.F. office.

I have a doubt about filling out FORM-19. In the form, for the 4th point where I need to enter the account number, I'm confused about what I need to fill in because it was given as "account no:...........DL. /". Can anyone explain to me how to fill in my PF account number in the mentioned column?

No revenue stamp is needed as at present all EPF and EPS transactions are made online. The PF office has also published a circular.

The DL is for region sign so you do not have to worry about it. You have to write the PF number as provided on the UAN site. Your PF number is also mentioned in your account slips.

In 10C, those who have not completed 10 years must withdraw their entire EPS, but as per the Table-D calculation. Those who have completed 10 years under the EPS scheme are eligible for a pension, but only after reaching 58 years. For them, the office has inserted a column in 10C for the scheme certificate. Here, you need to mention your wife's name, children's names as nominees, their age/DOB, and relation. Based on this information, the PF department will issue a scheme certificate that you must keep until you reach the age of 58 for pension eligibility. After that, your dependents become eligible for a pension.
