Dear Everybody,
Mine is a proprietorship firm and we have registered with ESI & PF so am not well conversant with the various regulations.
Recently one of my employee asked for FORM 37A for registering with the ESI hospital. Can anybody guide me about what it is and how am I to obtain that. I asked my accountant and he said he does not have a copy of it. So it would be very kind of you if anyone can mail me a copy at or just reply with an attachment.
