Questionnaire On OCTAPACE

Hi,
I am a Research Scholar (new to this forum) pursuing research relating Leadership with Organisational culture. I would like to use the OCTAPACE (developed by Prof. Udai Pareek) tool for my work.
Can anyone of you send me the questionnaire along with the brief scoring pattern and the validity/reliability of the scores?

u can refer to the book "Traning instruments for HRM " its by Udai parrek only........it has all the details that u want.......rite from the questionnaire to the scoring pattern etc...........
