Please help my friend to claim my EPF which is deducted from Mumbai office and he is now in Tamil Nadu. Kindly check the reliability of the attached form given by Mumbai Office asking for Cheque Leaf and other details. Is it mandatory to fill the form and return the same? If he has no cheque book then what to do?
