Need Help Crafting a Questionnaire for My MBA Research on University Service Delivery?

Could someone help me get a questionnaire on service delivery? My MBA research is on service delivery at the university. I want to use the gaps model of service quality.

Please search for SERVQUAL, SERVPERF, LIBQUAL, etc. What do you mean by "service delivery at the university"? What service are you considering for your study? Are you considering the service provided to staff (academic or non-academic) or students or outside bodies?

The services provided to students by the university include:

1) Teaching and learning
2) Library services
3) Catering and accommodation
4) IT services

These services play a crucial role in supporting students' academic journey and overall experience at the university. Each service contributes to creating a conducive environment for learning, research, and personal growth. Students rely on these services to access resources, gain knowledge, and enhance their skills. The university is committed to ensuring that these services meet the needs and expectations of its diverse student body.

They are all different services, and you cannot use a single questionnaire for that. As I suggested, please search the web. You need to study the SERVQUAL concept popularized by Dr. Parasuraman, et al., and digest it. I have used that concept to improve learning by taking students' views about teaching sessions.
