Subject - How to fill certain columns in Form 19 and 10C
Hi Sir,
I worked for a company from 2007 and relieved on March 2009 in Bangalore and pursuing higher studies now. I want to withdraw the PF money now but my ex-employer is not helping or no reply. I downloaded forms 19 & 10C and decided to get attested from the Bank Manager. I want to clarify my doubts in filling certain columns in form 19 and 10C.
1. Form 19
Contribution for the current financial Year:
Problem: What should I enter in the table because I resigned 2 years before on March 2009.
2.Form 19 & 10C
Name & Address of the Establishment in which, the member last employed:
Problem: My ex-company shifted to a new address after I resigned. But I worked in the old address only. So which address should I use it in the specified column as I am confused about that.
3. Form 10C
Are you willing to accept Scheme Certificate in lieu of withdrawal benefits:
Problem: What should I say? Yes or no?
Please help me!!

In current FY contribution NIL and to that support you attach the service certificate / experience certificate / relieving letter issued by your previous employer.

In your address column you mention the company's name and whatever address mentioned in your service certificate, you can mention in separate sheet about the shifting of office premises to new location. Further your establishment code alongwith your PF Number will show them all the details. You need not to worry much about it.

About acceptance of Certificate in lieu of Money if you tell yes they will issue a certificate. But if you tick No then they will pay the 8.33% whatever the emoluments available on your account.

You can directly submit the forms to concerned PF Commissioner office alongwith the Bank Pass Book first page, name and address proof, service certificate copy.

Never forget to mention your Mobile Number in the top of the application, so that you will get the SMS Alert about the cheque release and deposit details in Bank by EPFO.

Thank you very much for detailed reply. May god bless you with everything in your life for your help. I have few more queries before confirming the PF forms 19 and 10C. Could you please help me about that.
1. In which part of the Form 19(2 pages) & Form 10C(4 pages) should I get the attestation as some Bank Managers are doubtful. Or should I get attestation in all the pages of Form 19(2 pages) & Form 10C(4 pages).
2. Can I send the application through registered post(with acknowledgement) or should I directly submit in the Bangalore PF office as I am in other state.
