Can someone provide me with some inputs on how should i develop performance appraisals forms for my broking firm.

Hi,
You can apply different methods of appraisal - one to one menthod, 360 degree , observation rating method etc. First you need to analyse departmental needs with line manager and do audit for appraisal, it can be survey or feedback form by seniors /subordinates .
