When you are leading a safety meeting, focus on the following points in your presentation:

1. Total accident-free hours year-to-date.
2. All accidents and near-misses year-to-date statistics.
3. Additional safety information specific to your company.
4. Status of action items from the previous meeting.
5. Latest safety news from around the world.

I am unable to provide a sample presentation, but you can use this as a base to continue. This will be for presenting during the meeting. Additionally, after the meeting, you need to prepare the minutes of the meeting and circulate them.

Nice to see your contributions. Thanks for sharing.

First of all, I think there will be no problem for you to share such a document of your company in which even the names of your top executives are there.

Secondly, you may add the agenda of the meeting after the attendance part of your MOM. This can give an impression of what all things you have discussed in the meeting even if there are no points arising that need to be noted in the MOM.

One more point from the MOM is that you can avoid the "highlighted by" column. This makes no sense as it's understood that the point to be discussed arises from any of the forum members. There is no use in specifying it in the MOM; the key area to focus on is the result. If there is any specific reason behind it, please let me know.

One of the things I learned from DuPont is that each member of the Safety Committee should be given a responsibility. At each committee meeting, each subsection head should present a summary of the proactive HSE activities carried out, in addition to performance and audit reviews, as well as lessons learned from incidents and audits.
