Seeking Help: How Can I Create an Effective Performance Management Questionnaire for My HR Project?

Hello members and senior members, my name is Dhanya, and I'm doing my project in HR at Tata Teleservices. I've chosen "Performance Management System" as my topic for the internship. I'm planning to do a questionnaire on the effectiveness of the Performance Management System at TTSL. I would be really grateful if somebody helps me out with the questionnaire and analysis. Could somebody suggest some good ideas on how I could go about the whole thing?
