Confused About Filling Form 13 for PF Transfer? Need Help with Specific Columns!

hi all,
I just downloaded the form 13 for transfering pf from one employer to another
i see soem column like this
5.By whom the --- Regional PF commisioner Name of the P.F Trust
PF account
of the previous
estt is kept
can anybody ahelp what to fill under those two columns.where tofind those information for filling it up?
i am from chennai and this is my P.F office address

This is in reference to your posting about what is to be written in column No. 6 of Form 13. Firstly, you need to find out whether your company has its own P.F. trust or if it deals with a regional provident fund office for all P.F. matters. Once you have the details, you can fill in the column accordingly. Here is an example:

1. By Whom the P.F. Account of the previous employer is kept - RPFC (Nehru Place - Address of the P.F. Office)
A) Regional P.F. Commissioner at - Delhi
B) Name of the P.F. Trust - NA (In case the company does not have a trust)
