Agreement Between Client And Consultancy - DOC Download

Hi!
I am a HR Consultant, very new to the organization. I am working in a Hr consultancy. I was been asked to prepare an agreement which can be used between the Client and Consultancy for recruitment purpose.
Can someone please help me with this?

Dear Shikruti,
Find attached one model contract.
Add/Deduct SLA as per your MOU between you and your company.
If you don't have a MoU then add/deduct SLAs as per your telecon with the concerned persons in client companies.
