I agree with Mr. Nitin comment but its just a formality. According to law form 11 is not compulsory to submit in pf department. Form 11 will be attach with employee personal file with signature of employer & employee. No need to submit form 11 in pf deptt.

Form No.11: Declaration on EPF Subscription:
Form 11 under EPF scheme pertains to information provided by an employee while joining a
new employer, about his previous employments with name and address of employers along
with duration of service. In this form a new joinee also declares that whether he was
previously a member of EPF scheme or not. This form is to be kept by an employer and not
to be deposited with Sub/Regional commissioner. EPF inspector while inspecting records of
an establishment may ask for these forms to check the eligibility criteria.

Dear all,
we have total 200+ employees all employees are above 6500 basic. this form 11 is applicable for all. all employees are not want to the pf deduction. please clarify.

Form 11 will be required for UAN generation. This is part of Digital India and all benefits being linked to Account.

Dears,
The Employee has Joined in 2013, Now he want to claim PF.But in uan List that employee name was Changed. Whether We want to submit Form 11 for this Employee.
Please give Advise.
