I am doing my summer project on the topic "Effectiveness of Employee Induction Program and Enhancing the Same" in a manufacturing industry. For data collection, I need a questionnaire to measure the effectiveness of induction and orientation. Please help me out.

I would suggest that you create a questionnaire and send it for comments rather than asking for a questionnaire itself. This way, you will be able to learn how to frame questions, analyze data, and review results.
