How to register, pay and submit returns on PT regulary. Pls guide me

I want to know what is the rate of Gujarat Labour Welfare Fund For the Period Of Jan'12 To June'12.? If anybody has notification share it to us.

Dear Mr. Bhupendra
In Gujarat Employee's Share is Rs.3.00 and per employee Employer has to pay Rs.6.00.
Please deposit your contribution by 30th of this month.
I have also attached the LWF form for your reference.
