Can anybody tell me about the different types of PF forms and how to fill out this form?

Hi Kanchan,

You need to fill Form 13 if you want to transfer your PF to the new organization or Form 19 if you want a withdrawal of the total amount. Submit both the forms with the previous employer.

Form 12: A monthly return, Form 5: new joiners, Form 10: details of resignations during the month, Form 6A: annual return, Form 3A: annual return of individuals. For withdrawals, Form 9 and Form 10 are required. Hope this information will help you.

The Employees' Provident Funds and Miscellaneous Provisions Act, 1952:

The Act provides for the institution of Provident Funds:

1) Employees' Provident Fund Scheme 1952

2) The Employees Pension Scheme, 1995

3) The Employees Deposit Linked Insurance Scheme, 1976

Establishments, whether unexempted or exempted, have to comply with the Regional Provident Fund Commissioner.

The primary form all have to ensure is Form 2: Nomination and Declaration Form (subscriber and employer have to ensure).

Form 3: Contribution card for the currency period from... to... (for unexempted establishments).

Form 3A: Contribution card for the currency period from the 1st... to the 30th/31st... (for unexempted establishments).

Form 5: Return of employees qualifying membership

We were new to registering in EPF. All the challans are created on the E-sewa portal, but I want to know which form needs to be filled for all employees registering in EPF for the first time.
