A Survey To Complete My Project Work - DOC Download

Hi,
I have attached a questionnaire for parents. I will appreciate if as many of the forum members fill the questionnaire and email the completed questionnaire to [Login to view].
Once I receive all the questionnaire, I will be able to collate and complete my project report.
The profile of the participants in this survey is as follows:

• Parents of School/College going children
• Parents must reside in India

Dear Makara,
You need to clearly state as to what course you are doing and where, the purpose of the survey, how the responses will be used and what's the benefit to the responders.
Please search the web using Google and study how questionnaires are designed and formatted. If I remember I have given links in one of my previous posts.

I am undergoing the course from Global Open University. There are no personal details requested for in the questionnaire. Data collected will be used only for the project report and nothing more.
