Hi, I have attached a questionnaire for parents. I would appreciate it if as many forum members as possible could fill out the questionnaire and email the completed form to [Login to view]. Once I receive all the questionnaires, I will be able to collate and complete my project report.

Dear Makara, you need to clearly state what course you are doing and where, the purpose of the survey, how the responses will be used, and what's the benefit to the responders. Please search the web using Google and study how questionnaires are designed and formatted. If I remember correctly, I have provided links in one of my previous posts.

Thank you for your reply. I am currently enrolled in a course at the Global Open University. There are no personal details required in the questionnaire. The data collected will be used solely for the project report and nothing beyond that.
