Format Required For LTA

Hi Guys !
Can someone help me how do I mantain the data (softcopy) of LTA - actually to be claimed (entitled), LTA claimed till date and what after we paid the employee what is the balance amount which can be claimed by him / her. I mean the balalnce amount etc....

Even I am looking for one, or if anyone can explain the calculation for LTA not claimed to be given along with the Full & Final settlement.
What is the eligibility for claiming LTA.
How is tax exemption calculated on this??

As per your LTC system, how do your reimburse the said amount? Do you take into count the LOP day or NOT?
