Need Help Crafting a Questionnaire on Flexible Working and Absenteeism for My Dissertation Due Friday

My name is Obi, and I am studying Human Resource Management at a university in the UK. I am writing a dissertation on flexible working and how it has helped reduce absenteeism. I am worried about how to prepare the questionnaire, and I have to hand it in by this Friday. Could anyone please help me with this? It is very important.
