How Can I Withdraw My EPF from a Closed Consultancy in Another State?

Dear Sir/Madam, Kindly suggest to me - I was working through a consultancy agency in an organization in Mumbai five years ago. Now, I am working in Kolkata. The consultancy business in Mumbai has been closed, and I don't have a Payment Slip as it was not provided to us. However, PF was deducted, and I have my EPF number and the EPF Office address where the amount has been deposited. Kindly suggest how I can withdraw my EPF amount in this situation.

Dear Mr. Sanjiv,

You have to fill up the PF Withdrawal form which you will get on the PF site wherein the employer's seal and signature are required and then submit the same to the respective PF office. It can't be done individually alone.

Claim of EPF from Another State

Dear Sir/Madam, Kindly suggest to me, I was working for an automobile company in an organization in North Bengal three years ago, and now I am working in Jaipur for an automobile company. I have a payment slip from my previous employment as it was provided to us. I have my EPF number and the address of the EPF Office where the amount is being deposited.

Kindly advise on how I can withdraw my EPF amount in this situation.
