Appraisal Method For Manufacturing Industry

I need to set up an employee (staff) performance appraisal system in a small scale manufacturing industry.
Please advise as to which would be the best "appraisal method" to use. I already have the list of relevant KRA/KPA/KPI.

Can you mail your your KRA/KPI's etc., I will design one appraisal format and sent it to you. Give me your company details also.
I have done this for many firms and results have been fruitful.
